SINUSOIDAL CURRENT

 INTRODUCTION:

Sinusoidal current is evenly alternating sine wave currents having a frequency of 50hZ. It gives 100 pulses or phases in one second of 10 ms each, 50 in on direction and 50 in the other. It can be generated by reducing the voltage in 60 or 80 volts with the step down transformer.

EFFECTS OF SINUSOIDAL CURRENT:

The main action of the sinusoidal current is to to produce the muscle contraction. If it is applied continuously, tetanic contraction will occur and there is stimulation of both sensory and motor nerves which will produce tingling and needle like sensation. This is the prime reason for which the surging of current has been carried out. Once the current is surged it will produce rhythmic muscle contractions.
the pain relief occurs when sensory nerves are stimulated , by pain gait mechanism. Also when there is contraction of the muscles, this contraction has a massage like effect which not only improves the circulation of the blood butt also help in the reduction of edema  by improving the lymphatic drainage.

APPLICATIONS:

  1. Explain the procedure to the patient, check all the areas to be treated either their sensations are intact or not and remove all metals.
  2. Soak the electrodes in the warm water, place the electrodes pads upon the motor points which have to be stimulated.
  3. Pads must be held firmly by means of straps.
  4. Do not apply over the bony prominences.
  5. Current should be increased slowly and according to the comfort of the patient. if any side effects are reported, stop the treatment immediately.
  6. Stop the current slowly by slowly stopping the machine.
INDICATIONS:
  1. Moderate surging has an effect to cause contraction and relaxation of the muscles.
  2. Pain control
  3. Reduction of edema
  4. Rapid surging is beneficial in lower motor neuron lesion
  5. If sustained sinusoidal current is used, it will cause fatigue of reflex and muscle relaxation is obtained, so helpfull in muscle spasms and pain associated with it.
CONTRAINDICATIONS:
  1.  pacemaker is inserted
  2. Recent or nonunion fracture
  3. Recent scar after injury
  4. Over abrasion
  5. Hemmorhagic site’
  6. Near an abcess
  7. Any condition where muscle contraction causes a delay in the healing process.

BRONCHIECTASIS

INTRODUCTION: Bronchiectasis means abnormal dilatation of the bronchi due to chronic airway inflammation and infection. It is usually acquired, but may result from an underlying genetic or congenital defect of airway defences. CAUSES: Congenital • Cystic fibrosis • Primary ciliary dyskinesia • Kartagener’s syndrome (sinusitis and transposition of the viscera) • Primary hypogammaglobulinaemia Acquired • Pneumonia (complicating whooping cough or measles) • Inhaled foreign body • Suppurative pneumonia • Pulmonary TB • Allergic bronchopulmonary aspergillosis complicating asthma • Bronchial tumours CLINICAL FEATURES: ● Chronic cough productive of purulent sputum.  ● Pleuritic pain. ● Haemoptysis.  ● Halitosis. Acute exacerbations may cause fever and increase these symptoms. Examination reveals coarse crackles caused by sputum in bronchiectatic spaces. Diminished breath sounds may indicate lobar collapse. Bronchial breathing due to scarring may be heard in advanced disease. INVESTIG...

PARKINSON'S DISEASE

Parkinson's EtiologyParkinson's disease (PD) is a neurodegenerative disorder that mostly presents in later life with generalized slowing of movements (bradykinesia) and at least one other symptom of resting tremor or rigidity. Other associated features are a loss of smell, sleep dysfunction, mood disorders, excess salivation, constipation, and excessive periodic limb movements in sleep (REM behavior disorder). PD is a disorder of the basal ganglia, which is composed of many other nuclei. The striatum receives excitatory and inhibitory input from several parts of the cortex. The key pathology is the loss of dopaminergic neurons that lead to the symptom .  It is the seconds most common neuro-degenerative condition in the world after Alzheimer's. The condition is caused by the slow deterioration of the nerve cells in the brain, which create dopamine. Dopamine is a natural substance found in the brain that plays a major role in our brains and bodies by messag...
